We offer both face to face or telephone advice for pre-bookable (3, 7 or 14 days) and bookable on the day.

Primary Care Sheffield
Services offered by Primary Care Sheffield include;
- general contraception advice
- advice on complications with recent LARC fits
- pregnancy choices counselling and referral to Ward G1
- POP initiation counselling
- Patients can make appointments directly by calling 0114 226 8888.
Our clinical pharmacists offer a remote issue of EHC via the patient's own GP clinical system. Patients can self-refer to this service by calling 0114 226 8888.
PCS clinical pharmacists will:
- offer EHC/coil counselling
- for females aged 16+, prescribe EHC and issue remotely via EPS
- for females aged under 16, refer them to STH's youth clinic or an approved community pharmacy for a face-to-face appointment
- issue 3/12 POP under the recommendation of a specialist doctor or nurse
- book appointments for emergency coils
- Our specialists will continue to fit emergency coils and see patients with complications. While available resource permits, we will hold up to three sessions a week, on Mondays, Wednesdays and Thursdays initially at Sloan Medical Centre and Darnall Primary Care Centre.
Our approved community pharmacies are authorised to issue EHC free of charge, under PGD, to females aged 14-25. Pharmacies in Crystal Peaks, Heeley Retail Park and Meadowhall will come online shortly.

Population Health Management
People's health is changing, and care needs are evolving. People are living longer, but with more years in poorer health and it is increasingly common for people to have two or more long term health conditions. We know that health is determined by an interaction between individual characteristics, lifestyle and the physical, social and economic environment - the wider determinants of health.

Population health management and integrated care systems
The health and care system will need to be more joined-up and proactive in responding to that change. Integrated care systems (ICSs), place-based partnerships and integrated neighbourhood teams offer a renewed opportunity for the NHS to focus on prevention and integrated care: integration between primary and secondary care, physical and mental health and between health and social care.
Population Health Management (PHM) will be a core enabler and function of integrated care systems in helping drive a data led focus on person-centred care.
It can help local integrated teams to reduce health inequalities and offer targeted proactive, personalised, and preventative healthcare for every community.
The use of joined up data across local health and care partners and techniques like population segmentation and risk stratification can offer deeper insight into the holistic needs of different population groups and the drivers of health inequalities. Embedding this approach across all integrated care systems will transform the way we work and the way we care for people.

Long Term Conditions Service
A Long Term Condition is one that is unlikely to go away (like asthma or diabetes), and requires ongoing attention to keep their health in a good state. This is best achieved by a partnership between patient, practice and the wider support organisations. We know this involves considering health, social and self-management aspects of care.
We have been working on our approach for supporting people living with Long Term Conditions, as a team of staff, patients and our local community organisations. We are all listening and learning.
We are tailoring our approach depending on your health conditions and the measures that we take to indicate how you are getting on. We will proactively invite you for a planned annual review in the month before your birthday. You will get a letter (or email if you have registered), indicating your group:
