You must be available to receive a phone call and be available if needed to attend an appointment throughout the day
Please use an admin request form for doctors letters, referral information and test results
For prescription requests or queries please submit a medical request not administrative
DO NOT SUBMIT A MEDICAL REQUEST VIA THE ADMIN ROUTE BECUASE IT MAY NOT BE ACTIONED
Please do not use the link below for urgent or emergency requests.
The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays.
Please contact 111 during out of hours or 999 if this is an emergency.
Finally, if your problem is a rash, lump or bump please attached a picture along with as much information as possible.

Defaulting an Appointment
We ask that patients keep appointments or tell us as soon as possible if a booking becomes inconvenient. Defaulting appointments can be detrimental to you, the practice and other patients
You can either use the appointment confirmation text link to cancel or text us on 07493 362401
We operate a practice policy for patients who fail to attend their appointments.
If you fail to attend any appointment with a healthcare professional you will receive a warning letter reminding you of the importance of cancelling unwanted appointments. If following this letter you fail to attend a further 2 appointments (which also result in a warning letter) without cancelling it you will be sent a final warning letter.
If after this you fail to attend again this may result in you being removed from our practice list.
