You can contact us online to book an on the day or a pre-bookable doctor or physiotherapist appointment. However, all requests will be triaged by a doctor and the doctor will decide from the information you provide when you need to be seen and by which clinician.

Please read our booking information below for more detail.

Our online form can be used to book a doctor’s appointment, administrative issue, or prescription request/issues.

To book with a Nurse, Health Care Assistant or Phlebotomist please call the practice or call in, as a triage form is not required.

When you click on the link below, we will ask you a few short questions and get back to you during the day.

Please supply as much information you can to help the doctor decide when you need to be seen and by who.

Please go through the mobile phone verification after submitting your form to confirm your identity.

If thought to be needed to be seen on the day you must be available to receive a phone call and be available to attend an appointment throughout the day.

If we try to contact you and you fail to respond you may be asked to submit another form the following day as we cannot hold appointments all day.

Please use an admin request form for doctors letters, referral information and test results

DO NOT SUBMIT A MEDICAL REQUEST VIA THE ADMIN ROUTE BECAUSE IT MAY NOT BE ACTIONED

Finally, if your problem is a rash, lump or bump please attached a picture along with as much information as possible.

Defaulting an Appointment

We ask that patients keep appointments or tell us as soon as possible if a booking becomes inconvenient. Defaulting appointments can be detrimental to you, the practice and other patients

You can either use the appointment confirmation text link to cancel or text us on 07493 362401

We operate a practice policy for patients who fail to attend their appointments.

If you fail to attend any appointment with a healthcare professional you will receive a warning letter reminding you of the importance of cancelling unwanted appointments. If following this letter you fail to attend a further 2 appointments (which also result in a warning letter) without cancelling it you will be sent a final warning letter.

If after this you fail to attend again this may result in you being removed from our practice list.
