To book a same day or a routine pre-bookable appointment with a doctor or physiotherapist please read the following. To book with a Nurse, Healthcare Assistant or Phlebotomist please call the practice or call in, a triage form is not required.

When you click on the link to triage you will be asked a few short questions, the doctor will triage the form and we will get back to you the same day for a on the day appointment or within 3 days for a routine non urgent appointment.

If we try to contact you and you fail to respond, we’ll try you twice and if we don’t get to speak to you, you may have to visit the Walk in Centre.

Finally, if your problem is a rash, lump or bump please attached a picture along with as much information as possible.

Online access to your medical records

If you would like online access to your medical records please request at reception . You are required to provide two forms of I.D one with your current address and picture.

Online repeat prescriptions

Once registered for online services you will have access to request your repeat prescription. Requesting a repeat prescription will take us 48 hours.

Standard Operating Procedure / Protocol for use of Heidi AI Scribe

Heidi AI is a transcription software to aid the clinician by creating a note that can be copied into the patient record.
Key Points:
1. Patient should be asked for consent before using the software, and please document consent given. If a patient declines then please add on the front screen as a Reminder so that they are not asked repeatedly. There will also be information on the website and on noticeboards around the surgery informing patients that we may be using Heidi during consultations.
2. The clinician should consider with vulnerable patients such as those with dementia, learning disabilities or children whether they are able to give valid consent to use of the software and if in any doubt do not use it.
3. Please set up the system to delete all transcriptions automatically after 7 days or can be deleted by the clinician immediately after copying into the record. If you are unsure how to do this then please check / ask.
4. It remains the responsibility of each individual clinician to check that the transcription is accurate and reflects a true record of the consultation before pasting it into the medical records. This also includes any letters or other documents created by Heidi following the consultation.
5. Heidi will only be used by qualified and registered clinicians.
6. Heidi should not be used for clinical decision making.
7. It should only be used when consulting patients from Sloan Medical Centre not on other occasions or during hub sessions when we may see patients from other PCN practices.
