Mounjaro (Tirzepatide) Information Update

From 24 October 2025, GPs in the Sheffield area will be permitted to refer patients to Oviva for Tier 3 weight management for Mounjaro (Tirzepatide) injections to support patients in losing weight.
Eligibility for Mounjaro under the NHS will be extremely limited during the first year of rollout, with strict criteria designed to prioritise individuals most at risk due to their weight and related health conditions.
To qualify for the Oviva Tier 3 Weight Management Service, patients must meet the following criteria:
Have a BMI above 50kg/m (47.5)
Or
Have a BMI above 40kg/m (37.5) with four of the following five conditions:
- Hypertension
- Dyslipidaemia
- Obstructive sleep apnoea
- Cardiovascular disease
- Type 2 diabetes mellitus.
*Adjustment of 2.5 kg/m2 in people from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean ethnic backgrounds to ensure equitable clinical prioritisation and access to appropriate treatment. This also applies to people of mixed race who have one of these backgrounds.
AND
To ensure that patients demonstrate commitment to their weight loss journey, the guidance specifies that patients must have participated in a recognised weight loss programme, as defined with the guidance, in the 24 months preceding referral.
- A locally commissioned Tier two weight management service
- An equivalent NHS nationally commissioned Tier two programme (such as National Diabetes Prevention Programme, Digital Weight Management Programme, Type two diabetes Pathway to Remission)
- Attended a commercial weight loss service
- Primary care / Local authority health coaching, dietetic or practice nursing support to enhance diet/nutrition +/- increase physical activity +/- weight management
- Completed the 12 week free NHS Weight Loss Plan app
If you do not meet the BMI threshold and have at least four of the listed medical conditions, your GP will not be able to refer you to the service.
Patients should complete a triage request via our admin route on our website. We will then send you a referral checklist for you to complete to ensure you meet the referral criteria.
