To ensure we keep your medical records as secure as possible and do not release them to anyone not authorised we are introducing a new password system from 1st September 2019
Anyone asked to collect any information or documentation on your behalf will be required to have a password to do so.
Therefore before we can release any information you are required to set a password to your medical records.
Your representative will then be required to quote your password and D.O.B to enable the release of items such as prescriptions, fit notes, results, reports, letters etc.
Without a password and D.O.B we will be unable to release your information to a third party
